What should reasonable and customary be on most common procedures?

The answer to your question is complicated.
Procedures are assigned CPT codes. For example 99201 is a code for a new patient office visit for about 10 minutes. Then you need to consider the geographic location, and the training for the physician. All these things combined will arrive to an actual charge amount.
